At Alliance Integrative Medicine, we AIM to transform the health of each of our patients by creating a personalized wellness plan that leverages the best of conventional and alternative medicine to deliver meaningful outcomes. We call this Transformational Medicine℠.
Whether treating you for a specific condition or working with you to create a personalized long-term health and wellness plan, we use an integrative approach that looks at you holistically, as an individual with your own particular needs.
It all starts with learning as much about you as we can. To make your first visit as comfortable and efficient as possible, we ask you to review a few things before coming to the office.

To make an appointment with a team member, you will need to schedule a discovery call to see if this is the right fit and to discuss the best path of care.
Your new patient forms will all be completed on our patient portal. For a list of forms to complete, please review your appointment confirmation email.
To help us understand your medical needs, please bring copies of pertinent medical records and diagnostic test results or upload them to the patient portal prior to your appointment.
If you are requesting other doctors’ offices to send medical records, please allow ample time for them to comply with your requests. Please have records faxed to 513-342-5395.
If you wish for AIM to obtain your medical records from your doctor, please complete “Authorization for the Release of Medical Record Information” in the portal.
We need to know what medications — both prescription and nonprescription — you take. Please bring a comprehensive list of all drugs you take, including drug name, dose, prescribing doctor, and condition it treats. Please do the same for any over-the-counter drugs and supplements you take. If you prefer, just bring the bottles with you.
You can also add all of these on your patient portal in the respective sections.
Open communication with our patients is of primary importance to us. We want to address all your health questions and concerns, including issues that our new patient forms may not include. Please bring a list of your questions and concerns with you.

If you are unable to keep your appointment, please let us know at least 24 hours before your scheduled appointment time so we can offer this valuable time to another patient.
In the event of a late cancellation or missed appointment, you will be charged 50% of the appointment fee.
