Welcome to AIM for Wellbeing

At AIM for Wellbeing, we aim to transform the health of each of our patients by creating a personalized wellness plan that leverages the best of conventional and alternative medicine to deliver meaningful outcomes. We call this Transformational Medicine.℠ Whether treating you for a specific condition or working with you to create a personalized long-term health and wellness plan, we use an integrative approach that looks at you holistically, as an individual with your own particular needs.

How to Become a Patient

(1) Schedule a Discovery Call

To get started, schedule a discovery call, which will help us get to know you and your specific needs so we can determine the best path of care. If you decide to move forward with us, we will schedule your first appointment during the call.

(2) Set Up an Account in the Patient Portal

Once you have scheduled your first appointment, we will email you an appointment confirmation that includes an invitation to the patient portal.

(3) Complete New Patient Forms

You will complete new patient forms in the patient portal. For a list of forms to complete, please review your appointment confirmation email. Be sure to check out our Notice of Privacy Practices as well.

How to Prepare for Your First Appointment

(1) Bring or Upload Your Medical Records

Please bring copies of medical records and diagnostic test results or upload them to the patient portal before your appointment. If you request another doctor's office to send medical records, please allow ample time for them to comply with your requests and have them fax records to 513-342-5395. If you would like us to obtain your medical records from your doctor, please complete “Authorization for the Release of Medical Record Information” in the portal.

(2) Bring or Upload Your Medications & Supplements

We need to know your prescription and nonprescription medications. Please bring a comprehensive list, including drug name, dose, prescribing doctor, and condition it treats or upload this information to the portal before your appointment. Please do the same for any OTC drugs and supplements. You can also just bring the bottles with you to the appointment.

(3) Bring Your Questions

Open communication with our patients is of primary importance to us. We want to address all your health questions and concerns, including issues that our new patient forms may not include. Please bring a list of your questions and concerns with you. We look forward to welcoming you in person to our center. If you have any additional questions, feel free to contact us at 513-791-5521. We’ll do all we can to help make your first visit as comfortable as possible.

Your First Appointment

During your initial consultation, your physician at AIM for Wellbeing will carefully review and discuss your medical history as well as perform any needed physical exams. If you are a good candidate for our services, your provider will discuss the necessary lab tests and schedule your follow-up appointments.

The Patient Portal

Patients have access to message the AIM for Wellbeing provider team through our patient portal. However, if an email question or request is deemed by one of our staff to be too complicated or involved for a response through the portal, they may, at their discretion, require that the patient schedule a (paid) phone, telemedicine or in-person visit instead of responding via portal.

If you are NOT an in-state resident, your initial visit with a Medical Doctor at AIM for Wellbeing must be in person. After the first visit, all follow-up visits can be conducted by phone or virtually using our secure video channel if you choose.

If you live out of state, you can also establish care with our Registered Dietitian and Health Coach to get started on your journey back to health.

Cancellations

If you are unable to keep your appointment, please let us know at least 24 hours before your scheduled appointment time so we can offer this valuable time to another patient. In the event of a late cancellation or missed appointment, you will be charged 50% of the appointment fee.
